Position: Mental Health Therapist #297

Basic

 

Part Time
Position Summary: Provides on-going assessment, crisis intervention, individual, family, group therapy as well as case management. Develops and monitors service plans, monitors treatment progress, documents interactions and maintains clinical records and other reporting data for consumers with mild to severe mental and substance abuse illness, disorders and or disabilities. Coordinates with internal as well as external supportive services to deliver needed services to consumer. Collaborates with clinical supervisor and psychiatrist regarding treatment.

Skills and Ability:

Ability to provide treatment and case management in routine and crisis situations

Ability to read, analyze, and interpret related professional periodicals, journals, technical procedures or government regulations

Crisis Stabilization Services

 

Part Time
Position Summary: Float Pool Nurse to cover variety of shifts Sunday through Saturday depending upon unit needs.

Provides direct nursing service and supervise health related aspects of care. Provides prescribed medical treatment and personal care services. Evaluates and monitors clients for medication side effects. Coordinates medical care and intervention with staff and psychiatrist. Provides clinical assessment/problem identification, crisis interventions, individual, family and group therapy and case management. Develops and monitors treatment plans, monitors progress, documents significant interactions and maintains clinical charts and other reporting data.

Qualifications: Float Pool Nurse to cover variety of shifts Sunday through Saturday depending upon unit needs.

Licensed Practical Nurse licensed to practice in Colorado Required

Position: Behavioral Health Consultant #300

Integrated Care

 

Full Time
Position Summary: The Behavioral Health Consultant (BHC) is assigned to the NRBH/Sunrise Clinic, located at the Main Center. The BHC offers consultation to patients, and works closely with PCP's, psychiatrists, and therapists. The BHC provides screenings, assessments, short-term targeted interventions, intakes, as well as Triage and referral for long-term care. BHC is available for crisis intervention services in addition to case management. The BHC support activities designed to increase patient and provider awareness of the psychosocial aspects of medical illnesses. The BHC will be self-directed and have the ability to multi-task.

Qualifications: Education & Licensure:

• Masters or Doctoral Degree from an accredited College of University in a recognized behavioral health field and be either licensed or licensed eligible in that field (e. g. LMFT, LPC, LCSW, Licensed Psychologist, or pursuing one of these licenses )

• Valid Colorado Drivers License

• Bilingual Required


Skills and Ability:

South County (Ft. Lupton)

 

Full Time
Position Summary:
Provides on-going assessment, crisis intervention, individual, family, group therapy as well as case management. Develops and monitors service plans, monitors treatment progress, documents interactions and maintains clinical records and other reporting data for consumers with mild to severe mental and substance abuse illness, disorders and or disabilities. Coordinates with internal as well as external supportive services to deliver needed services to consumer. Collaborates with clinical supervisor and psychiatrist regarding treatment.

Essential Duties:

• Provides services to clients as assigned by Administrative/Clinical Supervisor
• Maintains client records in accordance with agency standards
• Provide/facilitate a wide variety of individual, family and group therapeutic and rehabilitative activities to achieve goals of services/treatment plan
• Conducts crisis intervention via telephone, and face-to-face treatment. Intervene and de-escalate crisis situations with clients. Assess risk of harm to self or others and arrange needed resources
• Observes clients in a variety of situations; Assess mental status of consumer; Assist clients in formulating treatment plans and goals; Perform case management services as needed to assist client to meet their goals as stated in their service/treatment plans; Refer clients/consumers to supportive services such as vocational, medical, social services, housing authority, etc
• Advocates for consumers in many different forums including at the federal, state and local court systems, with family, school system etc.
• Assists consumers in improving their well-being and quality of life by training in behavioral skills; educate consumers to make informed choices; develop consumer coping skills; help consumers to problem solve and negotiate tasks of daily living
• Maintains close and cooperative relationships with community agencies and other public agencies in carrying out treatment/service plans

Position: Behavioral Health Worker #302

Maxwell Center

 

Full Time
Position Summary: Participates as a member of the program team to implement the individual care plans for program consumers in accordance with the applicable policies, rules, regulations and directives of the organization. Facilitates consumer participation in their care plan by interaction with them as a source of emotional support, resource problem solving, and advocate. Assures security for consumers by monitoring and supervising the facility and the services delivered by the facility.

Essential Duties:
• Assist supervisory staff in assuring that consumers comply with program rules, guidelines and directives while they participate in all aspects of daily living and treatment within the facility and outside the facility when instructed
• Observe consumer behavior to identify change or indicators of special needs and report those observations to designated staff as they occur.
• Participate in the delivery of group care activities and serve as assistant to the clinical staff in treatment events as requested.
• Function as crisis intervention specialist when required to implement response plans for consumer acting out behaviors, for health emergencies, or for natural disasters.
• Maintain clinical and other operational records as required.
• Assist with meal service and participate with consumers during the meal time as indicated by consumer care plans.
• Assist in the consumer admission procedure including orientation, training, and other activities to maximize positive adjustment to the setting.
• Conduct house/community meetings.
• Assist in the consumer’s relationships with family and key others in a manner consistent with the consumers care plan.
• Provide transportation assistance when instructed to do so by supervisory staff.
• Assist in special outings, tours, trips and other special projects that are part of the program plans
• Assist in monitoring consumer’s use of medications and the management of those medications with all safety team by assisting in the monitoring of compliance in all emergency response procedures such as fire evacuation.
• Trained and competent restraint training.
• Demonstrate ability in use of time outs, token economy, therapeutic privacy, therapeutic physical restrain, and locked time out as behavioral interventions.
• Function as member of the facilities safety team by assisting in the monitoring of compliance with all safety regulations and by participating in the ongoing orientation and training of clients in all emergency response procedures such as fire evacuation.
• Follow all state and local requirements.
• Perform other duties as assigned

Qualifications: Education & Licensure:

Bachelors of Science required for MHW II & III from an accredited college or university, or 3 years relevant experience
Medication Aide Certification required (QMAP)
Valid Colorado Drivers License.

Experience:

No experience necessary but is preferred.
Demonstrated ability to plan and use effective interventions with consumers.


Skills and Ability:
