NEED OUR SERVICES?
Do you know or think you might need mental health services? Have you been referred by another agency to check things out? If so, call us at (970) 347-2120 in Greeley or (303) 857-2723 in Fort Lupton to request an appointment.
WHAT TO EXPECT
Upon your request for services, we will ask you questions to assist
in determining what kind of services
would be best given your circumstances. If you decide that you want to enter
into services, we will schedule an intake appointment.
At the intake appointment, we'll get more
details about what is going on with you, fill out required paperwork, and evaluate
what you tell us. Then you will be assigned a clinician/care coordinator who will
help you make a service plan and get you started with treatment. If it appears
you need medications, an appointment will be made for you to see a psychiatrist.
We will need a lot of information to learn how best to help you. We need to know as much as you can tell us about how you are feeling, what happened to make you seek services, and how long you have experienced symptoms. It is important for you to tell us about any history of mental illness in your family.
We will ask whether you have Medicaid or Medicare and for information about your insurance carrier. Be sure to bring your insurance, Medicaid or Medicare cards with you. If you don't have insurance, we'll find out if you need help to pay for treatment. To speed up the process, you'll want to bring proof of income.

and ask to speak with an emergency-services counselor. Emergency services
are available by phone 24-hours a day.
